Since 2021, state inspectors have cited Atienza Residential Care 60 times (32 serious, 28 minor) and confirmed 16 complaints across 46 visits. Among the most serious findings: a staff member became a legal power of attorney for a resident, which California law prohibits home employees from doing; inspectors found in 2024 that someone who had been barred from the home was present and issued an immediate fine; and video footage confirmed that a caregiver forcefully pulled a resident by the arms in 2024. A resident with dementia went missing from the home in 2023 and was found at a hospital, with inspectors citing the home for not having adequate safety measures for residents who wander. In 2023, inspectors also found that the home's rooms did not meet fire egress requirements for residents who cannot walk on their own, calling it an immediate risk to residents. More recently, in 2025, staff turned away an Ombudsman who came to check on residents, blocked a hallway with large water bottles, and several of a diabetic resident's medications were missing from the home's medication list. Many other complaints were looked into over the years and were not supported by the evidence.
