In August 2023, a resident with dementia left the home and was found at San Gorgonio Hospital; inspectors confirmed the home lacked required safety measures for residents with dementia. Fire safety was also flagged: in March 2023, inspectors found that rooms used by non-ambulatory residents did not meet fire exit requirements, a finding they characterized as an immediate health and safety risk. In October 2024, an excluded person was found at the home and the home was fined. In 2025, inspectors confirmed that a staff member had been blocking a hallway with water bottles, that a resident's medications were not listed on required medication records, and that staff turned away an Ombudsman at the door because they didn't know who the Ombudsman was. The state looked into many other complaints over the years; most weren't supported by the evidence.
