The most serious substantiated findings include staff neglect resulting in a resident being hospitalized twice within 1 week in August 2024 due to heat exposure; the resident was found outside in approximately 100-degree heat on both occasions, with his body temperature reaching 107 degrees Fahrenheit on the first incident and requiring CPR on the second. Additional substantiated complaints confirmed a staff member physically handled a resident with dementia roughly and provoked him in February 2024, then pushed and restrained that same resident against a wall in May 2025. In September 2025, investigators substantiated that staff failed to refill a resident's prescription and did not respond when another resident called for help. Inspectors also confirmed strong fecal and urine odors in multiple rooms alongside unsanitary floors and common areas in May 2025, a sanitation problem observed again during a November 2024 inspection. A staff member was removed from the facility in December 2025 for conduct deemed harmful to residents.
