The home contacted police, who later told the home the resident had been in a car accident the next day and died. Inspectors found that staff gave one resident the wrong dose of two medications: 100 mg of sertraline instead of 50 mg, and 5 mg of amlodipine instead of 10 mg. Staff also initialed that all residents' medications had been given through May 19, 2023, which inspectors called falsifying the records. The home wasn't recording the correct dose, month or year for any resident's medications, and inspectors described this as an immediate health and safety concern. At the August 2023 annual inspection, inspectors noted incomplete paperwork in two staff files. A separate 2022 complaint, about an unreported fall and beds without sheets, wasn't supported by the evidence. The annual inspections in August 2022 and August 2025 ended with no citations.
