The most serious substantiated finding involved a resident displaying inappropriate sexual behavior toward other residents on multiple documented occasions from 2024 into early 2025, including an incident in February 2025 in which a resident was found on top of a roommate; records show staff were aware of the behavior and the facility obtained medication to address it. A 2022 complaint was also substantiated after a staff member admitted to not transferring a resident to the toilet despite the resident's repeated overnight requests; that employee was subsequently terminated. State investigators responded to numerous additional complaints including allegations of medication coercion, staff retaliation, and withheld personal items, the majority of which were not substantiated.

The community features three care neighborhoods, 100% dementia-certified staff, the Nexus brain-health program, and on-site licensed nurses and a physician medical director. The home-like setting in a residential area gives it a quieter, more intimate feel than larger Silverado locations.
