In 2024, the facility was cited for failing to notify state regulators of COVID-19 positive cases within the required 24-hour window. Also in 2024, a verified complaint found the facility refused to readmit a hospice resident following hospital treatment for a stage 3 to 4 pressure wound without completing the required joint consultation process with the resident's family, hospice agency, physician, and licensing authorities.
